Niacinamide: what 2 to 5 percent actually does

Niacinamide is usually formulated at 4 to 5 percent, and the trials explain why that range is so narrow. In a dose comparison in 79 Japanese women, a 5 percent moisturiser reduced hyperpigmented spots against its own vehicle at 4 and 8 weeks while the 2 percent version did not reach significance, yet 2 percent was enough to lower the sebum excretion rate in 100 Japanese subjects over 4 weeks. Which percentage you need depends on the job, and the published efficacy work stops at 5 percent.

What the percentage on the label is doing

Niacinamide is the amide form of vitamin B3. It is water soluble, it feeds the cell's supply of the NAD coenzymes, and unlike nicotinic acid, the other form of vitamin B3, it does not cause flushing. In cosmetics it is mainly formulated at 4 to 5 percent, and that is not a marketing convention: it is the range the clinical work used, with endpoints measured at 2, 4 and 5 percent.

The clearest evidence on dose is a double-blind, split-face trial in 79 Japanese women with brown hyperpigmentation. One group used a 5 percent moisturiser on one side and the plain vehicle on the other, a second group used a 2 percent version the same way, twice a day for 8 weeks. At both 4 and 8 weeks the 5 percent side showed greater spot reduction than its vehicle. The 2 percent side did not reach statistical significance.

Above 5 percent the record thins fast. Tolerance has been tested there: at 10 percent niacinamide produced no stinging or flushing in test subjects, while a 5 percent product caused no irritation in single use and 21 day cumulative tests. What the published trials do not carry above 5 percent is an efficacy endpoint, so a serum advertising 10 or 20 percent is quoting a figure rather than a result.

Sebum and barrier, where 2 percent is enough

The sebum work is why 2 percent survives on labels. In Japan, 100 subjects were split into two balanced groups: 50 applied a 2 percent niacinamide moisturiser for 4 weeks and 50 a placebo, with the sebum excretion rate measured at baseline, week 2 and week 4. The groups started level and the treated group ended significantly lower at both points. In the United States, 30 white subjects ran a randomised split-face design for 6 weeks.

That group gave a different answer. Their casual sebum level, the oil sitting on the surface at a given moment, fell significantly; their sebum excretion rate, the rate of production, did not. One measure moved in each population, across 130 people in total. Expect a change in how oily skin looks, not a rebuilt sebaceous gland.

The barrier data start in a dish. Cultured human keratinocytes given 1 to 30 micromol per litre of nicotinamide for 6 days increased ceramide synthesis 4.1 to 5.5-fold, glucosylceramide 7.4-fold, sphingomyelin 3.1-fold, free fatty acid 2.3-fold and cholesterol 1.5-fold. Cells in a dish are not a face, so those multiples belong to the culture. What was measured on people is smaller: topical nicotinamide raised ceramide and free fatty acid in the stratum corneum and lowered water loss through dry skin, and a 2 percent moisturiser improved the barrier in rosacea. Pigment and fine lines, where 5 percent takes over

Niacinamide reaches pigment from an unusual direction. It had no effect on mushroom tyrosinase activity and none on melanogenesis in cultured melanocytes, so it is not a tyrosinase inhibitor in the way hydroquinone is. What it did was slow the delivery step: 35 to 68 percent inhibition of melanosome transfer from melanocytes to keratinocytes in a coculture model. That is interception rather than prevention, which is why the clock runs in weeks.

In the same 2002 work, 18 Japanese women with facial hyperpigmentation used a 5 percent moisturiser against its vehicle, and 120 Japanese women with facial tanning were assigned two of three treatments: vehicle, an SPF 15 sunscreen, or 2 percent niacinamide in that base. Both endpoints moved from 4 weeks. Against prescription comparators it holds its own without winning: in 27 melasma patients over 8 weeks, good to excellent improvement was recorded in 44 percent of the 4 percent niacinamide sides against 55 percent for 4 percent hydroquinone, side effects in 18 against 29 percent, and in 24 women treated on the underarms for 9 weeks, 0.05 percent desonide won, p equals 0.002.

For lines the anchor is a double-blind, placebo-controlled split-face trial. 50 white women with visible photoageing applied 5 percent niacinamide to one half of the face and vehicle to the other, twice daily for 12 weeks, images every 4 weeks. Fine lines and wrinkles, spots, red blotchiness and sallowness improved on the treated half, and elasticity improved on cutometry. Read the limits too: appearance scores over 12 weeks rather than biopsies over years, and several of these trials come from one company's group.

The vitamin C question, answered with the chemistry

The advice to keep niacinamide away from vitamin C starts with something visible. Put ascorbic acid and nicotinamide in the same water and the liquid turns yellow at once, because the two interact through charge transfer hydrogen bonds and form a non-salt complex at 1 to 1 stoichiometry. For years that colour was read as proof of mutual destruction. The chemistry says otherwise: the complex is now classed as a cocrystal, and pharmaceutical work makes it deliberately.

Why it is made deliberately matters. Ascorbic acid self-oxidises through the reactive hydrogens of its enediol group, and hydrogen bonding to nicotinamide shields them. A 2026 study produced vitamin C and nicotinamide cocrystals above 99 percent purity at a maximum yield of 85.2 percent, at 35 to 55 degrees Celsius and 80 to 90 bar, and the vitamin C in that powder showed no significant degradation after 24 hours at 80 degrees Celsius. Read it for what it is: a dry crystal made under pressure, not a serum. It shows the pairing is not inherently destructive, not how any one formula behaves.

The second half of the myth is that niacinamide converts into nicotinic acid, the form of vitamin B3 that does cause flushing. That conversion means hydrolysing the amide bond, which needs heat and acid or alkali rather than the conditions in a jar at room temperature. The effect the myth predicts has also been looked for on skin: at 10 percent, twice the usual cosmetic strength, niacinamide produced no stinging and no flushing. Formulators have meanwhile put both in one product, with 30 subjects using a gel of 2 percent ascorbyl glucoside, a vitamin C derivative rather than the acid itself, and 3.5 percent nicotinamide, and 18 subjects testing a formula with vitamin C, niacinamide, tranexamic acid and arbutin over 8 weeks.

What can genuinely go wrong is tolerance. An L-ascorbic acid serum has to be acidic, near pH 3.5 or below, and that stings on reactive skin whether or not niacinamide is present. Using it without wasting it

Niacinamide is water based, so it goes on early, after cleansing and before anything heavier; the rules are in the layering guide. Twice daily is what the trials used. Because the mechanism acts on melanosomes in transit, nothing visible happens quickly: 4 weeks was the earliest point at which spots separated from vehicle, and lines needed 8 to 12 weeks. Judge a product at 12 weeks, and treat single before and after photographs as photographs.

It is not a one off repair. In the 79 subject dose comparison, the spot reduction from 5 percent faded once use stopped, until it was no longer statistically different from the vehicle 42 weeks later. The trials that lightened tanned skin used 2 percent niacinamide inside a sunscreen rated SPF 15, the other half of any pigment plan: without daily protection, new pigment arrives faster than transfer can be slowed.

The tested tolerance margin is wide but not unlimited, since reactions differ between people and a finished formula holds more than one ingredient. Patch test on the inner forearm for 3 to 5 days if your skin reacts easily, add one product at a time if you have rosacea or eczema, and see a doctor for persistent burning, swelling or a spreading rash. Topical niacinamide is not on the usual pregnancy restriction lists, unlike retinoids, but other ingredients in the bottle may be.

Choosing and using a niacinamide product

  • Match the concentration to the job: 2 percent for surface oil, 4 to 5 percent for spots, tone and lines.
  • Treat any figure above 5 percent as a label number, since the published efficacy trials stop at 5 percent.
  • Give it 4 weeks before expecting any change in pigment and 12 weeks before judging lines.
  • Keep using it: the spot reduction from 5 percent faded to vehicle level 42 weeks after stopping.
  • Pair it with a daily sunscreen; the trials that lightened tanned skin used an SPF 15 base.
  • Patch test for 3 to 5 days and add one new product at a time if you have rosacea or eczema.

Can niacinamide and vitamin C be used together?

Yes. The yellow colour that started the warning is a 1 to 1 complex now classed as a cocrystal, and pharmaceutical work makes it on purpose because it steadies ascorbic acid, which showed no significant degradation as a dry powder after 24 hours at 80 degrees Celsius. That is solid state chemistry rather than a serum test, but it is the opposite of two ingredients destroying each other, and finished formulas have carried both.

Is a 10 percent niacinamide serum better than a 5 percent one?

The published efficacy trials for sebum, pigment and lines stop at 5 percent, so the extra percentage points buy an unmeasured claim. 10 percent has been tested for tolerance and produced no stinging or flushing, but tolerated is not the same as effective.

Will 2 percent do anything for oily skin?

In 100 Japanese subjects over 4 weeks, 2 percent significantly lowered the sebum excretion rate against placebo at weeks 2 and 4. In 30 white subjects over 6 weeks the surface sebum level fell but the excretion rate did not, so the effect looks real yet not identical across groups.

How long before hyperpigmentation looks different?

4 weeks is the earliest point at which the trials separated niacinamide from its vehicle on spots and skin lightness, and 8 to 12 weeks is the period over which the appearance endpoints were scored. It slows melanosome transfer rather than pigment production, so old deep pigment responds least.

Does the effect last if I stop using it?

No. In the 79 subject dose comparison the spot reduction from 5 percent faded during the regression period until it was no longer statistically different from the vehicle 42 weeks later, so this is maintenance rather than a course.

Is niacinamide safe during pregnancy?

Topical niacinamide is not on the usual restriction lists for pregnancy or breastfeeding, unlike retinoids, but the other ingredients in a finished formula may be. Take the full ingredient list to your midwife or doctor rather than relying on the front of the bottle.
