Hyaluronic acid: molecular weight and how to use it

Molecular weight decides how far a hyaluronic acid serum gets, and the numbers are narrower than the marketing. Raman micro-imaging of ex vivo human skin sections found that chains of 20 to 300 kDa pass through the stratum corneum while chains of 1000 to 1400 kDa do not, and in 76 women split across five 0.1 percent creams, about 15 per weight, only the 50 and the 130 kDa versions reduced measured wrinkle depth against the vehicle cream on the other eye after 60 days. The rest follows: low weights hydrate and need something over the top, high weights sit on the surface as a film, and neither one is an injection.

What the weight on the label means

Hyaluronic acid is one repeating sugar chain with no branches. What changes is length. Three synthase enzymes build it, and in intact tissue the native polymer usually runs above 1,000 kDa. Hyaluronidases cut it back down: HYAL-2 works specifically on high weight chains and leaves fragments of roughly 20 kDa, which PH-20 reduces to short oligosaccharides.

Cosmetic labels borrow that scale and use it loosely. There is no legal cut-off between low and high, so 300 kDa can be sold as low weight in one formula and as high weight in another, and terms such as hydrolysed or micro are descriptions, not specifications. Only a stated figure in kDa tells you anything, and most products do not print one.

The label also cannot tell you how brief this all is. In skin, hyaluronic acid has a half-life of less than 1 day, against 3 to 5 minutes in blood and 1 to 3 weeks in cartilage. Skin holds around 50 percent of the body's hyaluronic acid, most of it in the dermis, and the papillary dermis carries far more than the reticular dermis below it. Hyaluronic acid placed into the dermis from outside is cleared and degraded quickly, which caps what any topping up can do from the surface.

How far each size actually gets

The clearest penetration work used Raman micro-imaging, which identifies a molecule by its own spectrum inside transverse sections of skin, without labels or dyes. Those sections come from skin removed from the body, not from a living face, so this is an ex vivo result and not a clinical one. Three sizes were compared. Hyaluronic acid of 20 to 300 kDa passed through the stratum corneum; the high weight material at 1000 to 1400 kDa did not. One author worked for a hyaluronic acid supplier, which does not undo the imaging but belongs on the record.

Read what it does not say. Crossing the stratum corneum is crossing the outermost layer, not arriving in the dermis where fibroblasts make hyaluronic acid. Nothing between 300 and 1000 kDa was applied at all, so the cut-off people quote sits in a gap with no measurement in it. No published study appears to have followed a topically applied chain into the dermis and shown it staying there, and claims about serums that rebuild dermal hyaluronic acid are running well ahead of the measurements.

Smaller is also not automatically better. Size governs biological behaviour: polymers above about 1,000 kDa are anti-angiogenic and immunosuppressive in tissue, while smaller fragments act as distress signals and potent inducers of inflammation and new vessel growth. Nobody has shown that this matters at 0.1 percent in a cream, and the 2024 trial below reported no adverse reactions, but it is reason to treat the race to ever smaller fragments as an open question rather than an improvement.

What the trials measured, and on whom

The molecular weight comparison people quote is a periocular study in 76 women aged 30 to 60 with visible crow's feet. Each woman was randomised to one of five creams at 0.1 percent, at 50, 130, 300, 800 and 2000 kDa, so roughly 15 women carried each weight. The cream went around one eye twice daily for 60 days with the vehicle around the other. Hydration and elasticity were measured at baseline, 30 and 60 days, and wrinkle depth read from negative replicas. All five improved hydration and overall elasticity against the vehicle; only the 50 and the 130 kDa creams improved mean roughness and maximum roughness, the two replica measures of wrinkle depth. Four of the seven authors worked for the ingredient suppliers that made the hyaluronan, so this is not an independent trial.

A double blind randomised trial in 2024 put the same question to dry skin. 36 nursing home residents in Jakarta, aged 60 to 80, had three lotions assigned to three separate sites on the leg: low weight hyaluronic acid, high weight, and the vehicle. After 4 weeks the low weight site read 56.37 arbitrary units of skin capacitance against 52.37 for high weight (p equals 0.004) and 49.01 for the vehicle (p less than 0.001). Transepidermal water loss and the symptom score did not differ between any of the three, and no adverse reactions were reported.

Both trials are small, short and surface-level. 36 people, 4 weeks and one leg is a pilot-sized answer; 76 faces over 60 days is about 15 per weight and an appearance endpoint scored from replicas, not a biopsy. Hydration readings rise when corneocytes swell with water, and that swelling is the likeliest reason fine lines look shallower without anything changing underneath. Judge a serum at 4 to 8 weeks, and treat single before and after photographs as photographs.

Dry air, damp skin and the layer on top

The winter warning is familiar: in dry air a humectant pulls water out of the deeper skin and leaves you worse off. The direct evidence for that in living human skin is thin, and the claim is repeated far past its data. Anatomy argues against it, as an inference rather than a measurement: the lipids made in the stratum granulosum block diffusion of water through the epidermis, so water held in the dermis is not free to cross on demand and the two behave as largely separate accounts. Neither side has been tested in a trial, so treat both as reasoning.

What has been measured is the effect of what you put over the top. In a pilot study on living human skin explants treated daily for 9 days, water loss fell by 15.6 percent under non cross-linked high weight hyaluronic acid and by 27.8 percent under a cross-linked form, while low weight hyaluronic acid raised water loss by 55.5 percent. Epidermal water content rose 7.6 percent over untreated controls with the cross-linked material. These are explants rather than faces, from a small study run with a filler manufacturer, so read the direction and not the decimals: a low weight serum is a humectant, and it wants an occlusive or emollient moisturiser sealing it in.

Damp skin is the weaker half of the advice. No published trial compares damp against dry application, and the trials above gave no such instruction, so applying within about 1 minute of washing is a sensible habit rather than a proven step. Why a serum is not a filler

The molecule is the same and the object is not. A filler is a cross-linked gel, injected through a needle or cannula into the dermis or deeper. Cross-linking is what makes it last: the native polymer has a half-life of under 1 day in skin, while bonding the chains to each other slows the enzymes to months. The risks, the reversal with hyaluronidase and the red flags of a blocked vessel are in the filler guide; none apply to a bottle.

Wording such as filler serum or needle free describes a surface effect that is real but temporary, and fades as the film washes off. Nothing there deposits volume, so if lost volume is the concern, a serum is the wrong tool.

Topical hyaluronic acid is unusually well tolerated. The 2024 dry skin trial recorded no adverse reactions in any of its three groups over 4 weeks, the periocular trial reported nothing on tolerability, and there is no pregnancy restriction of the kind that applies to retinoids. That says nothing about the other ingredients in a finished formula, and a humectant does not repair a broken barrier on its own; the signs of one are in the barrier guide. Patch test on the inner forearm for 3 to 5 days if you react easily, and see a doctor for stinging that lasts, spreading redness or swelling rather than working through the bottle.

Getting the most out of a hyaluronic acid serum

  • Look for a stated figure in kDa; without one, low and high weight are marketing words with no legal definition.
  • Apply within about 1 minute of washing, while the surface still holds water, so the humectant has water to hold.
  • Put an emollient or occlusive moisturiser over it, the step with the measured numbers behind it.
  • Expect surface hydration by about 4 weeks and softer fine lines, not volume and not a rebuilt dermis.
  • Ignore percentages on the front: the periocular trial used 0.1 percent creams twice daily.
  • Patch test for 3 to 5 days if your skin reacts easily, and see a doctor for stinging that lasts or spreading redness.

Does low molecular weight hyaluronic acid really penetrate deeper?

Deeper than high weight, yes, but not as deep as the advertising suggests. No trial compares damp against dry application, so this is reasoning rather than evidence. Applying within a minute of washing gives the humectant surface water to hold and costs nothing, while the moisturiser over the top is the step with numbers behind it.

Is a hyaluronic acid serum the same thing as a filler?

No. A filler is a cross-linked gel injected into the dermis, where the bonds between chains slow enzyme breakdown from under 1 day to months, while a serum works on the outermost layer and washes off. A serum cannot replace lost volume, whichever molecular weight is printed on it.

Which molecular weight should I buy?

A mixture is the sensible default, since in the 76 woman trial, about 15 women per weight, all five weights from 50 to 2000 kDa improved hydration and elasticity while only 50 and 130 kDa moved wrinkle depth, and on explants the high weight form cut water loss by 15.6 percent. If a product states no figure in kDa at all, judge it on how your skin feels after 4 weeks.

Is it safe in pregnancy or on sensitive skin?

Topical hyaluronic acid carries none of the pregnancy restrictions that apply to retinoids, and the 2024 trial recorded no adverse reactions in its 36 elderly participants over 4 weeks. The 76 woman periocular trial reported nothing about tolerability either way, and the other ingredients in a finished formula are a separate question, so take the full ingredient list to your midwife or doctor.
